A More Deliberate Approach to Psychiatric Medication Decisions

Psychiatric medications can be beneficial and, for some patients, essential. But circumstances change. Diagnoses evolve. Risks and benefits shift. For eligible Iowa patients, Dr. Monsalve reassesses ongoing indication, benefit, burden, adverse effects, withdrawal risk, relapse risk, concurrent treatment, and patient goals before recommending continuation, adjustment, gradual reduction, substitution, or discontinuation. Iowa tapering and deprescribing services currently concern non-controlled medications.

Tapering is not limited to antidepressants. Evaluation may include antidepressants, antipsychotics, mood stabilizers, anticonvulsants used psychiatrically, non-controlled medications used for anxiety, sleep, or ADHD, and non-controlled medications prescribed off-label for psychiatric symptoms.

A non-controlled medication used off-label for anxiety, insomnia, mood symptoms, behavioral symptoms, or another psychiatric purpose can be reassessed without implying that off-label prescribing was inherently inappropriate.

When several psychiatric medications are involved, the more important question may be how the entire regimen fits together. Changes may need to occur sequentially rather than simultaneously.
