Global Psychiatric Second Opinion
Verigrate Health provides focused psychiatric second opinions to patients, families, and treating clinicians outside jurisdictions where Christian S. Monsalve, M.D., a board-certified psychiatrist, can establish ongoing clinical care. An advisory review is intended to clarify a complicated psychiatric history, not to create an international treatment or prescribing relationship.
Consultations may be scheduled directly. What a review can address depends on the patient’s location, applicable law, clinical circumstances, and the role of the local treating team — and Verigrate does not represent that ongoing care or prescribing is available in every jurisdiction.
Clinical and Legal Scope
Ongoing Clinical Care and Advisory Review Are Not the Same
The distinction is substantive. Ongoing care involves an established treatment relationship over time. A second opinion is a focused review designed to inform understanding and discussion with the clinicians who remain responsible for treatment.
Ongoing Clinical Care
A continuing physician–patient relationship may include evaluation, treatment planning, prescribing when clinically appropriate, and follow-up. It is available only when Dr. Monsalve is legally able to establish and treat the patient in the relevant jurisdiction.
Advisory Second Opinion
A focused review may help organize diagnostic questions, prior treatment, medication strategy, tapering or deprescribing concerns, and medical or metabolic considerations. It does not transfer prescribing responsibility from the local treating clinician.

What an Advisory Review May Address
The purpose is not to produce a generic protocol. It is to identify the questions that matter most in a complicated clinical history and provide a clearly reasoned framework for the patient and local treatment team.
Diagnostic complexity
Questions about competing diagnoses, incomplete explanations, or a treatment history that has become difficult to interpret.
Prior medication trials
A structured review of what was tried, why it was changed, what appeared helpful, and where adverse effects or uncertainty remain.
Psychopharmacologic strategy
Consideration of medication burden, sequencing, interaction risks, and reasonable alternatives to discuss with the treating clinician.
Tapering and deprescribing questions
Questions about whether reduction is clinically appropriate, what complicating factors deserve attention, and what should be discussed before a change is attempted.
Treatment alternatives
A review of reasonable options, the uncertainties around them, and questions to bring back to the clinicians providing care locally.
Medical and metabolic context
Relevant physical health, sleep, metabolic factors, substance use, and medication effects when they materially shape psychiatric assessment.
Clinical Responsibility
Jurisdiction, Treatment, and Prescribing
An advisory review does not authorize Verigrate Health to provide ongoing treatment in a jurisdiction where Dr. Monsalve cannot legally establish a physician–patient relationship. It also does not replace emergency, hospital, residential, or locally supervised care when those settings are clinically necessary.
Local clinician responsibility
Where an advisory consultation is considered, the patient’s local treating clinicians remain responsible for treatment decisions, prescribing, monitoring, and implementation. Any recommendation must be interpreted within the patient’s medical context and the laws governing local care.

Consultations are booked directly — there is no application step and no waiting on a reply. When scheduling, note the patient’s location, the role of the local clinical team, and the central question. Detailed records are shared afterwards through the secure workflow rather than at booking.
Patients
Individuals seeking a focused review of a complicated diagnosis, medication history, tapering question, or treatment strategy.
Families
Families may book a review where consent, confidentiality, decisional authority, and the patient’s clinical circumstances are appropriately addressed.
Treating clinicians
Physicians and psychotherapists may book a focused review to support diagnostic or treatment planning, with patient authorization.
